This form is to be completed by applicants of JV Garage Door, LLC who have completed an application and have been approved to move forward to the interview process. The application and interview process consists of four stages which include: Resume Review; Application and Review; Written Aptitude Test and Initial Interview; Technical Aptitude (on-the-job) Test and Final Interview.
 
At the time you have progressed through the next two stages of the application and interview process, an offer of employment may be extended to you on a conditional basis of the results of your required background and drug test.
 
The following form is to request a time to complete the first stage of the interview process: Written Aptitude Test and Initial One-to-One Interview.
